MEMBERSHIP CATEGORIES AND FEES (CAD)

ACTIVE Member ($100/one year; $150/two years): any individual person who conducts research or practices in any field of health care

ASSOCIATE Member ($ 50/one year): any other individual person who supports the goals of the Institute (students with a current ID may become an Associate Member)

CORPORATE Member ($500/one year): any organization who supports the goals of the Institute

CRISH membership application involves 2 steps:

1. Submitting your payment through Paypal
2. Completing a brief online membership registration

Payment

CRISH uses Paypal to receive payment.  If you are unable to use Paypal, we also accept check and money order (in Canadian dollars).  Please fill out our member survey form and send by fax, e-mail or mail.  We will assist you with the registration.
